Near the end of a busy shift, a colleague quietly tells you they wrote in the notes earlier that a patient had been reviewed and was comfortable, but they had not actually gone back to see the patient yet. The patient currently appears settled, but a nurse has just mentioned that the patient seems more short of breath than before. Your colleague asks you not to mention the documentation issue yet because they are about to review the patient now and do not want to look bad in front of the team.
